Multiplex improvements

* Retire underlying SockJS connection when last channel is closed
* Logging (requires source change to enable--uncomment the contents
  of the debug() function)
* Ensure early channels (opened before SockJS connection is
  established) that are closed before the connection is open, stay
  closed
