The "no appellations for some counties" error resulted from the fact that while defining MARC 21 mapping rules I did not predict that a publishing house can give two locations as the "place of publishing".
This turned out to be quite difficult to fix because the subsequent place ID's had to be iterated from 0 separately, and they were found in exactly the same type of element (subfield code a).
Finalyl I introduced an attribute in the mapper which makes it possible to force it to reset the iteration for one element after leaving another one.
