Add pressure BC and mass flow rate integral output

Simple tests with a pipe prescribing a pressure difference and computing
mass flow rates at inlet and outlet, which are equal and opposite (due
to always outwards pointing normals), module numerical errors.

The pressure BC prescribes the density and pressure at a surface.
